Housing FormLoading...MNU is, by definition, a residential university. In support of this aspect of the University's character, the Board of Trustees has established a policy requiring most students to live on campus. The policy is that all unmarried students under the age of 22, not living with parents, must live on campus if they take more than 11 semester hours of classes. Exceptions to this residence policy are administered by the Housing Committee. All new MNU students, whether living on-campus or commuting, must complete this form. The purpose of these laws is to remove barriers that prevent qualified individuals with disabilities from enjoying the same opportunities available to persons without disabilities. A student with a disability who wishes to request a reasonable accommodation must contact the Director of the Academic Success Center at 913-971-3387 or asc@mnu.edu.MNU's complete policy regarding student disability accommodations may be found at mnu.edu/ADA.Vaccinations/ImmunizationsIt is the policy of the State of Kansas that incoming students residing in student housing be vaccinated for meningitis or sign a waiver refusing receipt of the meningitis vaccine. All students who will reside in university housing should have one Meningococcal SCWY (MCV4, not Meningococcal B) at age 16 years or older. If you received the Meningococcal SCWY prior to your 16th birthday, a booster will be necessary to ensure adequate protection and is due by the end of August (fall semester) or by the end of January (spring semester).Have you received the meningitis vaccine?YesNoDate of VaccinationDate of VaccinationJanuaryFebruaryMarchAprilMayJuneJulyAugustSeptemberOctoberNovemberDecember123456789101112131415161718192021222324252627282930312000200120022003200420052006200720082009201020112012201320142015201620172018201920202021202220232024202520262027202820292030203120322033203420352036203720382039204020412042Would you like to request a waiver?YesVaccination WaiverPlease read the Centers for Disease Control educational material regarding the hazards and risks of meningitis disease and the meningococcal vaccine at cdc.gov/vaccines.
